According to the Association of American Medical Colleges (AAMC), the U.S. faces a projected shortage of 17,000 to 42,000 specialists, including radiologists, by 2033, with rural imaging centers already feeling the strain. Effective January 2026, CMS permanently revised its definition of direct supervision to include immediate availability via real-time, two-way audiovisual technology. The updated framework covers Level 2 diagnostic tests, including contrast-enhanced CT scans, MRI studies with contrast, and other imaging procedures requiring physician supervision.
Previously, physicians were required to be immediately available onsite during these studies. The current rule allows the supervisory role to be fulfilled remotely through real-time audio and video communication, provided CMS requirements are met, and the supervising physician remains immediately available throughout the procedure.
ContrastConnect notes that remote supervision requires real-time audiovisual communication, qualified onsite personnel, and proper documentation. Audio-only connections do not meet CMS requirements, and facilities are advised to maintain contingency plans for connection failures. Records should document the supervising practitioner, availability, and technology used during each service.
